Consents Form

The Medical module Consents Form feature is optional database enhancement used to generate and print consent for release of information forms.

  NOTE: Consent information is only collected for contacts and support services contacts if an optional consents feature has been turned on in your AWARDS database.  If you don't see the consent options during contact and support services contact data entry and would like to, please download and complete the Contacts portion of the Face Sheet - Implementation Request Form, and then submit it to requests@footholdtechnology.com.

Required Permissions 

Because the Consents Form feature is optional, it is not automatically available in all databases.  When requested (by contacting the Help Desk) it is turned on behind-the-scenes in AWARDS by Foothold Technology, and is available to users with the following permissions:

Program Chart Access
Display Any Chart Records Buttons
Display Chart Records Medical Button

  NOTE: Permissions are assigned using the Permissions Maintenance feature.  If you do not have access to that feature and need a permission listed here, please contact your supervisor or your local Help Desk for assistance.

Printing a Consents Form 

 

To print a consent for release of information form, complete the following steps from the AWARDS Home screen:

    1. Click the Program drop-down arrow in the upper right-hand corner of the page and select the program associated with the client for whom a consents form is to be printed.
    2. Click Charts from the left-hand menu, and then click Medical.  The Medical fly-out menu is displayed.
    3. Click Consents Form.  The Consents Form Client Selection page is displayed.
    1. Click the Client drop-down arrow and select the client for whom the consents form is to be printed.

  TIP: If the consent form to be printed is for a former (discharged) client, first check Roster Archives beneath the client selection option.

  1. Click SHOW FORM.  The consents form is displayed on the Consent for Release of Information page.

  2. Use your web browser's Print option to print the form.

The process of printing a consents form is now complete.

The following frequently asked questions provide details on the most commonly asked about pieces of the Consents Form feature.

Where does the consent information information come from?  Can I add to it?

The consents information is pulled from contact information entered on the client's face sheet and within the Contacts feature in the Medical module if consents information has been entered for those contacts.  It is also pulled from the support service contacts information entered on the face sheet and within the Support Services Contacts feature in the Medical module when consents information has been entered for a contact there.  Beneath this information on the consent form are blank lines in which additional consents information can be entered.
