Medical Info

The Medical module Medical Info feature is used to maintain client medical information records, including known medical problems, contra-indicated medications, special conditions, and special dates.

Required Permissions 

Use of the Medical Info feature requires the following permissions:

Program Chart Access
Display Any Chart Records Buttons
Display Chart Records Entitlements Button
HIV/AIDS Data Access (optional - to access/record HIV information as part of medical info records if the program has been configured to allow it)

  NOTE: Permissions are assigned using the Permissions Maintenance feature.  If you do not have access to that feature and need a permission listed here, please contact your supervisor or your local Help Desk for assistance.

Working with Client Medical Info

 

To enter, update, or delete a client's medical info, complete the following steps from the AWARDS Home screen:

    1. Click Charts from the left-hand menu, and then click Medical.  The Medical fly-out menu is displayed.
    2. Click Medical Info.  The Medical Info index page is displayed.

    1. Click the Program drop-down arrow in the upper-right corner of the page and select the program associated with the client whose medical info is to be worked with.
    2. Click the Client drop-down arrow and select the client whose medical info is to be worked with.

  TIP: If the medical info to be worked with is for a former (discharged) client, first click the Archives toggle to the right of the client selection option.

The Medical Info page refreshes to display the client's medical information in report mode.

    1. Click the Edit icon to display the page in data entry mode. 
    2. At this time add, edit, or remove data from the text boxes on this page as necessary. 

  TIP: For more information on each field/option on the data entry page, see below.

    1. Click SAVE.  The medical info additions, changes, and/or deletions are saved and the Medical Info confirmation page is displayed.

  TIP: To make additional changes to medical information at this time, click the edit icon on the action bar at the top of the page to return to the data entry page.

  TIP: To view a read-only report version of the client's medical info, click the view icon from the action bar.  To view a report of aggregate medical info for multiple clients and programs, the fields contained in this feature are available for reporting on the Demographics ReportBuilder and Medications ReportBuilder.

The medical info data entry process is now complete.

 

A Closer Look @ Fields & Options 

  NOTE: A gear icon  next to a field or option in this list indicates that it is either optional (and not automatically available in all AWARDS databases), or that some aspect of it (for example drop-down selections or its status as required/not required) is configurable. To request some or all of the configuration options detailed here, please contact the Help Desk.

Contra-Indicated Medications

In this text box, enter any contra-indicated medications that the client should not be taking.

HIV Fields / Options 

When configured to display, the following HIV fields/options are displayed at the bottom of the page when you have the "HIV/AIDS Status Data Entry" permission:

  HIV Status - Click this drop-down arrow and select the client's HIV status.  The selection options are configurable; default choices include "CDC-defined AIDS," "HIV+," "HIV-," "AIDS Status Unknown," and "HIV Status Unknown."

  HIV Test Date - In this field, type or make changes to the date on which the HIV test was given (using mm/dd/yyyy format), or select the date using the date picker drop-down arrow.

  HIV Verification Date - In this typed type or make changes to the date on which verification was given (using mm/dd/yyyy format), or select the date using the date picker drop-down arrow.

  Risk Categories - Click the checkbox next to each of the client's applicable risk categories.

 IMPORTANT! Be aware that data recorded using the above fields/options only displays on related ReportBuilders to users with the "HIV/AIDS Status Data Entry" permission AND the report is being generated for a single program - OR - the report is being run for a group of programs, all of which are set to include these variables. If a program group is selected where not all of the included programs display these variables, they will not be shown in the report, regardless of permission.

In this text box, enter a description of any known medical problems the client has.

Special Conditions

In this text box, enter a description of any special medical conditions the client has.

Significant Dates

In this text box, enter a description of any special or significant dates related to the client.
