Import Templates Guide

The tables below detail the specific data variables collected in each ImportTools template, along with notes on how each should be formatted.  Those variables with a red asterisk (*) next to them are required.

When filling in any of import template spreadsheets with the data to be imported into AWARDS, keep in mind the following important guidelines:

  • Leave the header row of the spreadsheet with the column names in it as is; it is a required part of the import file.

  • Remember to periodically save your work.  While you may change the file name when doing so, be sure to continue using the original .XLSX extension (until directed otherwise in the Validation and Import Process).

  • Do NOT delete any columns from the import template, even if the corresponding data is not required or applicable to the program in question.

  • Enter data carefully and thoughtfully.  Editing and cleaning up the data on the import spreadsheet is much more time-efficient than doing so after that data is imported into AWARDS.


Client Identifiers

Record one row of data for each identifier.  If a client has multiple identifiers, there must be a separate row for each.

Data Variables Notes
Client Identifier * 9-digit AWARDS ID.
Unique Identifier *  Must be entered EXACTLY as configured in AWARDS.
ID Number *  Accepts alphanumeric values.

Client Roster

Record one row of data for each client in a single program.  If a client is currently enrolled in multiple programs, there must be a separate row for each enrollment.  All demographic data in each of those separate rows should identical; otherwise AWARDS will not know to create a single Client ID for that individual, as it should.

Contact Log

Record one row of data for each supportive services checklist contact record.  If a client has multiple records, there must be a separate row for each.  Conflicting records (those where contact start time and duration indicate an overlap with an existing record) cannot be imported.  Records with the same date, time, and client as an existing record will be imported but appended to the existing record rather than added as a new record.

Diagnoses

Record one row of data for each diagnosis.  if a client has multiple diagnoses, there must be a separate row for each.

Identifier ID *

An ID number as entered under Entitlements > Client Identifiers in AWARDS.  Only required if a Client ID is not specified.  When an Identifier ID is specified, a valued Identifier Type must also be entered.

Identifier Type *

A valid identifier type corresponding to the Identifier ID and entered EXACTLY as configured under Entitlements > Client Identifiers in AWARDS.  Only required if a Client ID is not specified.  When an Identifier Type is specified, a valid Identifier ID must also be entered.

Diagnosis Date *

MM/DD/YYYY format.  Future dates are not allowed.

Onset Date

MM/DD/YYYY format.  Future dates are not allowed.

End Date

MM/DD/YYYY format.  Future dates are not allowed.

Condition *

Accepts alphanumeric values.

ICD-10 Code

Diagnosed By

AWARDS employee ID of a staff member who meets one of the following criteria (when one or both optional features are turned on in your AWARDS database):

- Has the "List as Prescribing/Diagnosing Physician" permission
- Is specified as an "Eligible Provider" under Human Resources > Credentials

The ID number can be pulled from the Employees ReportBuilder in the Human Resources module, or from the DataBridge.

The "Update" action should be used when making changes to an existing diagnosis record.  The "Insert" action is only used when creating a new diagnosis record that does not already exist in AWARDS.  When using this action, only records with an "ICD-10" Code Set are accepted.

 


Employees

Record one row of data for each employee.  Because the work supervisor component of the import requires the supervisors to exist in AWARDS already, employee imports should be done in groups, hierarchically, starting with the top of the organization chart and working down.

Incidents

Record one row of data for each incident.  Basic requirements are noted below; however, any other values configured to be required in AWARDS incident records are also required in import file.

If a "yes" value is entered for "Close Existing Record," the import process checks for any existing program status records for the client.  If it finds an active record (one without an end date), an end date is automatically entered as part of the import.  The end date used is based on how the agency has configured the Program Status functionality, and may be either the same as the "Start Date" of the new record, or the day prior.  For information on configuring this aspect of the Program Status functionality, please refer to the Program Status portion of the Face Sheet - Implementation Requests Form.


Referral Agencies

Record one row of data for each agency.

Data Variables Notes
 Admission Referral Source  Yes or No.  "Yes" makes the imported agency available in the Intake/Admission module as a referral source agency.  A "Yes" value is required if Outgoing Service Referral is set to "No."
 Outgoing Service Referral Yes or No.  "Yes" makes the imported agency available in the Services - Individual module's Service Referrals feature as an agency for outgoing referrals.  A "Yes" value is required if Admission Referral Source is set to "No."

Service Referrals

Record one row of data for each service referral.  if a client has multiple service referrals, there must be a separate row for each.

Data Variables Notes
 Client ID *  9-digit AWARDS ID.  Only required if an Identifier ID and Identifier Type are not specified.
 Identifier Type * An ID number as entered under Entitlements > Client Identifiers in AWARDS.  Only required if a Client ID is not specified.  When an Identifier ID is specified, a valued Identifier Type must also be entered.
 Identifier ID * A valid identifier type corresponding to the Identifier ID, and entered EXACTLY as configured under Entitlements > Client Identifiers in AWARDS.  Only required if a Client ID is not specified.  When an Identifier Type is specified, a valid Identifier ID must also be entered.
Agency * Accepts alphanumeric values.  Must EXACTLY match the name of an "Outgoing Service Referral" agency within the Referral Agencies module.
Contact * Accepts alphanumeric values in "First Name Last Name" format; for example, "John Doe."  Must EXACTLY match the name of a contact for the specified Agency within the Referral Agencies module.  "Unknown" is also allowed.
Referral Date * MM/DD/YYYY format.  Future dates are not allowed.
Program Name * Must be entered EXACTLY as configured in AWARDS.
Purpose/Need * A valid purpose/need entered EXACTLY as configured in AWARDS.
Disposition Date * MM/DD/YYYY format.  Future dates are not allowed.
Disposition Status * A valid disposition status entered EXACTLY as configured in AWARDS.
Placement Outcome A valid placement outcome entered EXACTLY as configured in AWARDS.
Follow-up Date MM/DD/YYYY format.  Future dates are allowed, but may not exceed five years from the import date.
