Deleting a Progress Note

To delete a progress note, complete the following steps:

  1. From the left-side navigation menu, select Charts > Services-Individual > Progress Notes.

The Progress Notes Data Entry page will display.

  1. Select the Client from the drop-down menu. Note: If the client is a former (discharged) client, click "Roster Archives." 
  2. Continue. The Individual Progress Notes Editing Index page will display.

This page includes all the existing progress notes written for the selected client that fall within the editing window date range shown at the top of the page. Progress notes with corresponding contacts log records are indicated by an asterisk (*).

  1. Select the radio button next to the note that you want to delete.

Note: A solid bullet next to a note you wrote previously indicates that it has been locked and cannot be opened for deletion. If necessary, locked notes can be unlocked by users with the "Unlock Progress Notes" permission.

Note: A lock and key icon next to a note indicates that it has been electronically signed, preventing deletion.

Note: Progress notes written by other users are unavailable for deletion unless you have the "Surrogate Data Entry - Progress Notes" permission.

  1. Continue. The Progress Note Composition Settings page will display.

NOTE: If the note to be deleted is a service plan linked note, the Service Plan Note Linkage page appears after clicking CONTINUE in step 5. Click CONTINUE again to view the Progress Notes Composition page. 

  1. Click Delete at the bottom of the page. A Confirm Delete? page will display.

  1. Click Delete. The progress note and contacts log record (if applicable) will be deleted and the updated Individual Progress Notes Editing Index page will display.
