Viewing a Progress Notes Report

You can view a read-only report of progress notes based on a variety of criteria. You'll find this report in the Reports module. 

  1. From the left-side global navigation menu, select Reports

  1. The Reports Index will display. Select the correct program in the Program drop-down menu, and locate the Progress Notes Report. Click on the Progress Notes Report

  1. You can limit the report to only include notes by certain writers using the Writer drop-down selection menu. The default for this menu is "All Writers". 
    1. Note: The Writer selection list is initially limited to writers with caseloads in the selected program. Click Expand Writers List if you'd like the writer selections to include all users with chart access who have written a note for someone in the selected program in the last 90 days. Keep in mind that in divisional databases, you can not see users in another division - even with the Expand Writers List. 
    2. Note: If the report is for a former (discharged) client, the Writer selection must be set to "All Writers." 
  2. Select the Client whose notes need to be included in the report. The default is "All Clients." 
    1. Note: If the report is for a former (discharged) client, first click Roster Archives. When Roster Archives is selected, the Writer option must be set to "All Writers." 
    2. Note: If a specific writer was chosen in the Writer drop-down, select "Worker Caseload" from the Client drop-down to view only those progress notes written by that individual for the clients assigned to them. 
  3. Select the Date Range. Keep in mind that the maximum length of time for the report is a two-year period.
  4. Further designate which notes you'd like to include in the report by checking off one or more of the following:
    1. Merge With Group Notes
    2. Include Provider Appointment Information
    3. Include Program Appointment Notes
    4. Select Individual Note Sections (Individual Notes and Note sections are available for printing separately from the full report.)
    5. Insert Printing Page Break After Every Note
  5. You can view the report on all progress notes, or you can select the All Progress Notes drop-down menu and limit the report to specific note types (Service Plan Linked Notes, General Chart Notes, Admission Assessments Notes, etc.)
  6. You can use the Keyword Search field to include in the report only those notes containing specific text. 
  7. When finished, click SHOW NOTES.
  8. The Progress Notes Report will display on the Individual Progress Notes page.
    1. Note: If Select Individual Note Sections was checked, click "View This Note" for any note section to open it individually in report mode.
    2. Note: If Include Provider Appointment Information was checked, then all Provider Appointment Information will display on the report even if the notes do not contain the text that was entered in the Keyword Search field. 
    3. If the Service Plan Linked Notes Only option was selected, the Progress Report will display on the Service Plan Linked Individual Progress Notes page. Click the Bill? checkbox next to each progress note reporting on a contact that is to be billed to Medicaid. Click UPDATE SELECTIONS. The billing selection information will be saved and the updated Service Plan Linked Individual Progress Notes page will display.
